I just read the quest hand in text. Man, even NPCs hate Silithus!
Breadcrumb steps:
1) Complete all Swamp of Sorrows Quests.
2) Complete all Silithus Quests.
3) Turn in Warchief's Command: Blasted Lands! to Okrilla in Deadmaul Hold in Blasted Lands. Do not pick up any other quests.
4) Travel to Silithus and pick up Call of the Warmatron. Take the portal from the mage and turn in to Okrilla at Deadmaul Hold.
5) Travel to Stonard in Swamp of Sorrows and pick up Okrilla and the Blasted Lands.

Description
Our venerable warmatron, Okrilla, has sent me to the far reaches of the world in search of suitable heroes that would join our cause. Are you interested in strengthening the Horde's grasp over the Eastern Kingdoms? Then take this portal to the Blasted Lands and report to Okrilla at Dreadmaul Hold!
