TheWoWDBAnother player and I were both doing the skull throwing; I killed the mob and stayed at 3/5 Play with Deathfang. Once I ran out of the quest zone and back in I could toss the skull again.

You have to kill the skeletons and eventually the toy drops. You give it to Deathfang and then you're asked to throw the skull 5 times (the dog will fetch it and you'll have to take it from him)
You only need one skull.
You play fetch with Deathfang like you would with any other dog.Possibly the best world quest ever. ♥
