Throught the quest chain you get back all the time to the same 4 caves/rooms.
But there is 5th where 3rd Altar is placed.
Find stone, half round balcony with pyre on it looking over at the huge ship-gate into the mountain.
There is path down the cliff on the left of the balcony... go down and you will find one more room with the Altar

Description
Its grip weakens, if only briefly. We must claim this place anew! Flopping, nightmarish horrors have infested the caves amongst the cove. Take the sacred blessings. Venture forth into the dark and find their altars. Purge their blasphemous constructions from our lands!
