if you are looking for [npc=171834] you will find him behind the pillar where the minimap locates him, i uploaded a screenshot.
Once it asks you to open a rift, it took some time for the button to show up for me. Also make sure to collect the spheres when fighting the boss, they increase your damage.
no, he doesnt appear sometimes. at all, i relog, i reload, i dont get him showing up

Pins are the quest’s own POI points from the current retail client, projected onto the in-game zone map. The game marks one blob as the quest’s turn-in; the rest are objective areas. Points that fall outside a map’s window are dropped rather than guessed at.
Objective areaIn-game waypoint: /way #1533 67.4 29.9
TheWoWDB/way #1533 67.4 29.9Map coordinates of the pins above. An area the client draws as a shape is given by the centre of its points.
Quests whose map markers the client places closest to this one on Bastion, nearest first. Distances are straight lines between the markers.
Flight points closest to the quest’s first objective area marker on Bastion:
Straight-line distances in yards between the quest’s own map markers (QuestPOI) and the client’s flight-point positions (TaxiNodes); flight paths and terrain are not modelled.
First seen in build 12.0.7.68887 (Midnight patch 12.0.7, the oldest build in our datamine).
Steps, storylines and map placement from the client’s QuestLine, prerequisite and QuestPOI data; neighbouring quest text is Blizzard’s own (quest API), shortened to its opening sentences.
Completing this quest counts toward this achievement.
