i was looking for an npc to start this quest but couldnt find one. the way you get it is by entering phase walking (theres a portal just to the northwest if you need it), drop down to the cave and kill the gorge creeper mobs. they drop an item that starts the quest. i suggest avoiding the large elite worms patrolling. they hit pretty hard at current average gear levels. if you are geared then feel free.
edit. i suggest grabbing the quest [Untethered Potential] at the flight path nearby. the creepers drop the item you need for that quest as well.

Description

Covered in slime and saliva, the journal bears a partial entry of a previous Phase Diver. <Day 27 Untethered Space is much more vast than I expected, with monsters stronger than I have seen on K'aresh. There is a nearby cave I will explore tomorrow. It is brimming with Phase-Altered Essence. This could bode useful for better understanding the ecology out here.> Researcher Essarae would be very interested in this.
