Must have completed Darkheart Thicket: Nal'ryssa's Sisters (quest chain starts with The Felsmiths) for [item=124499] to drop. Eye drops from a felslate deposit.
Similar quests include:

Lyrelle's Right Arm which is started by mining [item=124500] from a felslate seam and

Ondri's Still-Beating Heart which is started by mining [item=124501] from the corpse of a felslate creature.

So the lesson we learn from rank 2 mining is: jamming some ore into your arm is great, but don't gouge out your eye and replace it with a chunk of rock, that's just crazy talk.

Got this today. 19 days played at 110.
There are nightmare stories out there about people farming herbs and nodes not getting gathering quest drops.
Some forms of quests require certain strange thresholds to be met, like waiting a couple of days after looting something. They are otherwise rare, unaffected by mining skill level, and require patience if you attempt to commit to farming some.

Description

Oddly enveloped within the felslate deposit is a humanoid eye. It quivers with life. You get the sense that it would blink at you if it had an eyelid. A mining expert should take a look at this.

I was not the only miner in my family. I was the youngest of four sisters. I say "was" because the other three are gone. Two of them went too far. They overloaded their bodies with felslate and succumbed to the will of Sargeras. They are more demon than elf, now.

You blink and rub your eyes, but they do not deceive you. This is a woman's arm, completely enveloped within a felslate seam. The arm is bloodless, but still twitches. Fel-colored streaks remain where veins once were. Felsmith Nal'ryssa will certainly want to see this.
