It is possible to walk back and take a bone griffon to get back to Acherus. In this case you can runeforge your new blade before fighting with Patchwerk. When you are done runforging, simply open the Death Gate, click on it and continue with your quest chain.
This allows you to skip going back to Acherus from Stormwind/Ogrimmar for runforging and jump straight to the Dark Portal instead.
Today, I somehow managed to hearth out of the DK starting area and into Stormwind before finishing the starting quests. I was on this quest, but couldn't interact with any NPCs and couldn't use my Death Gate ("can't do that while pacified").
In order to return to the Ebon Hold area to finish my starting quests, I had to ride from Stormwind east to Redridge Mountains. There, the flight master at Lakeshire was friendly and intractable to me, and I was able to take the flightpath back to Acherus: The Ebon Blade.
Thank goodness for that because I was just about to delete this toon and make a new one...

Description
