To start this quest kill Torturer LeCraft at 70,72.

I did this quest with the initial levelling wave through Northrend, as such I didn't have tap on the mob after I finished torturing him for information, so never actually looted him. If you have completed the quest, you can come back after to rekill him, and he will indeed drop the item, if you're a completionist.
