I must say that on the quest suckage scale, this one is right up there near the top. The quest area is very small, the mobs that drop the quest items are very specific and on a somewhat long respawn timer. If you're unlucky to be doing this quest on a realm that's even remotely active you'll be running back and forth A LOT.
It took me almost 30 minutes to do this... every class that had a distance closer or instant ranged attack pretty much sniped them right in front of me.
Stupid, stupid quest...
From Brewnall Village, you'll find them just a hint North West from there. Almost due West of the lake.
The Leper Gnomes like to run, which will typically mean that they come back with a friend, so do what you can to kill them before they get too far. Also, they like to hit you with "Decayed Strength", which reduces your strength by 2.
For those with Cosmos, or Titan Panel and like coordinates, I started finding Leper Gnomes around 26,43 approaching from the South.
After you complete the quest, be sure to stand back and watch Razzle Sprysprocket's invention kick into action. It's pretty funny.

What to do
Description
I should go back to Gnomeregan and continue researching a cure for irradiated gnomes. But, I stayed behind in Kharanos because the town is my home and I wanted to help defend it from the Frostmane attacks. I've been working on maintaining and repairing the steam tanks. For some reason, Frostmane scavengers have taken a liking to the gyromechanic gears I need to complete the repairs. They're all over the Frostmane Front to the southeast of town. Will you help me get those gears back?

Pins are the quest’s own POI points from the current retail client, projected onto the in-game zone map. The game marks one blob as the quest’s turn-in; the rest are objective areas. Points that fall outside a map’s window are dropped rather than guessed at.

Before this quest

- Forced to Watch from Afar
The cataclysm drove the troggs to the surface in hordes, forcing the Frostmane trolls to flee for their lives. Unfortunately, they've decided to settle right on our doorstep and make trouble for us.
After this quest
Taken from the storyline's own step order.
- Pushing Forward
Most of the mountaineers and steam tanks in Dun Morogh are battling along the Frostmane Front, which is approaching us from the southeast. We can barely hold the trolls back, let alone advance into their village and take on their leader.
