The fragment of revelation is a lightly-highlighted interactable rock with an orange symbol next to the chests in the treasure room at the end of the delve. So, if you're like me, assumed that the quest was just to complete the delve and left the instance after grabbing the chests - sorry. Got to get back in there again.

Once you've left the delve, take the immediate upper path to the left and the cave exit is "behind" you as you come out. Don't run forward looking for an exit, the cave system is kind of annoying with no mounted option.
