You obtain this quest after completing Crystal Eyes.

/way Highmountain 38.37, 61.32 Cave Entrance

Inside, you need to find Gelmogg and defeat him.

Once you enter the cave, you need to take the first right, follow the path and took the wooden stairs on 36.80, 59.43, then right again.
Gelmogg stays on bottom of the room, and on the middle of the fight Dargrul will appear stunning both Gelmogg and player (like he does every time you fight Gelmogg), but you don't need to fight with him. He will talk a little bit, and than he runs away.

Dargrul yells: Are these ronir giving you too much trouble, Gelmogg?
Dargrul yells: Perhaps I misjudged your worth...
Dargrul yells: When you're done with this distraction, meet us in Riverbend.

Once you defeat Gelmogg, you will obtain Ancient Highmountain Necklace.

To all people looking the low ilvl on the item... don't. It is one of the BiS items to Timewalking content, as those 3 gem slots are badass, even if other ones might give primary stat baseline. There is simply no other neck item with equal chance of slots, the closest one might be the 'chance' drop from Lei Shen, in ToT, that has only 2 slots. And remember you can put in low gems in them, tbc ones are still best since if you simulate cata and higher ones, you'll see that you lose soo much in the stats that these get their spot (I see even MoP gems, giving 10 in primary stat to their expac or higher, but lower the content's level, and you'll get below that).
