TheWoWDBAt first I had difficulties finding her, as the question mark was appearing in the middle of the hall, but she was not there. If you are already 60, Lady Moonberry can be found at 49, 52, just outside Night Fae hall.

If you level through the story and then switch to Threads of Fate just before dinging 60, and then complete this quest AFTER dinging 60, she moves outside the tree. The mini-map will show her question mark still inside, but that cake is a lie. She's outside the tree waiting to be spoken to.
Dinged 60 just as completing this quest cannot find NPC to hand in does it disappear once 60 grrr
