You have to complete both The Dark Heart of Nazmir and The Throne of Zuldazar on the same character. This quest is not account wide.
You cannot, for example, complete Zuldazar on your main character, and Nazmir on an alt (on the same account), and pick up the quest on your main (who has not yet completed Nazmir).Confirmed by GM Llirdray when I contacted support about not being able to pick up the quest:
to get that questline, you need to finish both Nazmir and Zuldazar on the same character- these questlines are not account wide.
You need to have completed The Dark Heart of Nazmir and The Throne of Zuldazar
You should then be offered Blood Gate, if you don't get Blood Gate and you've done Ticks already you won't get the Breadcrumb any more so don't worry :).

Description
We just received word that the Northern border is under heavy attack by an army of blood trolls. King Rastakhan has called for reinforcements. You are more familiar with that threat than any of us. Go and do what you can to help them.
