For me, when the Hammer wouldn't show in my Action Bar, I just used the Exit Vehicle button and talked to the NPC again.

The hammer will appear when you get near a bell, when it appears just click it. It will disappear again until you approach another bell.
If you can't see the hammer, disable addons and reload UI, if that doesnt work then abandon quest and accept it again
So...any interesting theories on why Kosmas simply didn't go up there and threw his hammer by himself? I fail to see the importance of me beng dragged around like a raggdoll in the air throwing his hammer for him.
Perhaps this is all inception and while we believe that this is us saving the afterlife, it really is just an afterlife tailored to our needs to be the heroes of the story. In our little Valhalla we get to keep saving the world, even though it's just a theatre.

Description
My heart is filled with dread. If it truly is Lysonia who leads them now... No. What is important is that we restore the great vespers. So long as their mighty voices are corrupted, all who hear them will suffer as this temple does. It is only a matter of time before all of Bastion succumbs. They will no doubt try to stop you.
