Ran into a situation where I had to log off as soon as soon as the Matron Mother starts to summon the portal, but it never fully summoned. (Kids... right?) When I logged back in the portal was nowhere to be found with no appropriate dialogue options.

Simple Fix: Abandon Quest and Re-Accept. There's no penalty and she'll summon the portal straight away.

So, not paying attention, I died. When I revived I freaked out because the barrier was back up and I couldn't get in! I ran back down to talk to the Matron Mother, in case she could let me in, and she wouldn't talk to me. Then I noticed another stairway on the other side. I ran up that way and it was not blocked. So if you die, just go around the other way and you can continue the scenario.

Legion Remix:
I encountered a bug where the "fight Akama" objective did not complete after the battle. The issue occurred because after battle I chose the second dialogue option with Akama. When I replayed the quest and picked the first option, the objective completed as expected.

We must discern their force structure, patrol schedule, new defenses, and what Akama eats for breakfast. <Belath grins.> I suggest you send our broken warriors to handle this.
