To find [item=159157] travel to 60.0 39.5 spot, there's a library. Pass through it.
You'll get into small inner yard, the rod is soaring above magic fountain.
Interact with it once to make mobs spawned. After killing you finally can obtain it.

I present At the Edge of Madness quest chain. Part of Stormsong and Dance.

1. A House Divided

2. The Storm Awakens
[npc=135534] - Rally 12 Defectors. They are hide in houses ( you geting 3 ) and single around houses in Sagehold. Be careful to not pull Umbral Hulk ! [npc=136498] will move on north

3. A Clean Slate
AUTO START - This Quest i starting Auto but you need to have started The Storm Awakens ( I done them bouth is same time ). With out nothing will pop up. Just kill mobs and destroy things in Sagehold.

4. Twisted Intentions
[npc=136053] - You need to kill 2 mobs: [npc=135457] and [npc=135506]. Bouth in Sagehold one in southwest other on northeast.

5. The Abyssal Council
[npc=136498] - Kill [npc=135985] He is in chamber northwest. [npc=136339] will be wait for you behind bridge on east side.

6. Oathbound
[npc=135517] - She is in mid of Sagehold. You need to get [item=159157] northwest. [npc=136497] will be wait for you behind bridge on east side.

7. Eye of the Storm
[npc=136497] - You need to escort [npc=136497] you will be attack 5 times and at the end she'll sacrifice so you can escape .... sad

8. A Bridge Too Far
[npc=136933] - Stand on middle and use spell on your extra action bar. You will made bridge :D.

9. Lost and Forgotten
[npc=136933] - Cross bridge and kill mobs to get to 100%. You will have company :D.

[npc=136933] - Destroy 3 tidemother's: [npc=137705], [npc=141762] and [npc=137706] all in Tempest'S Edge.

11. The Final Ascent

12. Storm's End
[npc=137506] - After accept will start animation. Then kill [npc=137197]. [npc=137691] will wait on cliff. After finish this quest you will reach part of Stormsong and Dance.

Description

I took oaths, damn it. We all did. When Lord Stormsong sent the first orders... well, you saw what happened. What matters is that our oaths are to our people, not Lord "Sells His Soul". You won't find him here - he's deep in the Shrine by now, and no outsider will make it across the channel alive... Not without a Tideguard to open the way. <Victoria locks eyes with you.> The Rod of Tides is the key to the front door, and it's kept here. Retrieve it and I can get you to him.
