You need to kill the waves of adds that spawn after clicking the fragment, until a bigger one appears. The quest only progresses after you defeat that bigger one.
I ran away right away and thought it was bugged 😄
Hopefully this helps someone!

Where to go

Pins are the quest’s own POI points from the current retail client, projected onto the in-game zone map. The game marks one blob as the quest’s turn-in; the rest are objective areas. Points that fall outside a map’s window are dropped rather than guessed at.

At last, <name>. With the promise of this fragment's power, and my mastery over the illidari ways, I will be ready to fight the shadowguard and any other threats the void throws at us. Leona spoke of a cave near Shan'dorah where the void energies suffusing K'aresh converge.

  • ChaosK'aresh · Level 80

This is it, <name>. Place the K'areshi fragment above Adarus, and I will channel the void lure containing Ramon'ta's void essence into the shard. Adarus is powerful and determined, but we're attempting something both experimental and dangerous. I'll ask you to be vigilant.
