If you change faction and Exarch Maladaar not appear;
Go Jorune Mine on Talador.Speak to [npc=78513] and complete the last few quests you need to do there as well.
Once this is done, head to Fort Wrynn in the north east and find [npc=79618]. She'll offer you the quest Speaker for the Dead and this will direct you to Exarch Maladaar.

What to do
Description
It is good to see you again, champion. Demons are assaulting Auchindoun and we are barely standing our ground against them. We need to push our front lines forward lest our losses reach critical mass. If your blade is with us, shove it into these foul creatures' throats.

Pins are the quest’s own POI points from the current retail client, projected onto the in-game zone map. The game marks one blob as the quest’s turn-in; the rest are objective areas. Points that fall outside a map’s window are dropped rather than guessed at.
