This quest is given by Malfurion on top of Nazj'vel. Malfurion appears there when you complete the objective of "The offering to Azshara". However he only stays there for about 1 min after the mini event ends. He actually despawned before I took this quest and the only place I could find him was at eye of the vortex.... however he wont give this quest from that location.
I found that if you repeat the event for "the offering to Azshara" by killing the 4 darkscale priestess on top of Nazj'vel at the green portal, then the mini event between Queen Azshara and Malfurion repeats itself and you can still get the quest from Malfurion here.
Hope this is of help to anyone else who finds Malfurion despawning before they take the quest.

This quest is started by a "spawned" NPC [npc=34422] during an event from the previous quest The Offering to Azshara.
If you didn't pick up the quest in time. Malfurion will despawn after about 30 sec. Simply redo the event with the horn, by killing the NPCs and the event will repeat, you can then pick the quest again.

What to do
Description
The ancients of the grove have come to our aid, {name}. I can see through their eyes even as we speak. The Twilight's Hammer has been forced to hasten their ritual and have thus only managed to resurrect but a remnant of dread Soggoth. Destroy it before it's too late, {name}. I am needed elsewhere. Return to the druids at the Grove of the Ancients when it is all over they will direct you further.
