[npc=215233] drops [item=212595] ONLY while you are on the quest Raging Rifts. I was doing Sojourner of The Ringing Deeps on my alt that did not complete the main campain or the chain leading to Raging Rifts and i did not get the shard on that one leaving "Rampage at Nibelgaz Mine" part of the achivement incomplete. I came on my main (the one that did not skip the main campain) and picked up the chain leading to Raging Rifts from [npc=219042] strting with The Nibelgaz Refinery. I hope this helps other people in the same situation. I also submited 2 screenshots one from the addon called BTW Quests but idk if they got approved.

    The elementals are coming from inside that cave. We only made it part of the way in before needing to retreat. I could see some flickering within, some strange glowing thing that looked almost like a tear in space from which the elementals poured forth!
