[item=193853] can be fished up from a [npc=203183], which is in zskera-vault, in The Forbidden Reach. Its location is random, and will be behind one of the doors, each of which needs a [item=202196] to open.

These keys are now bind on account, and you no longer have to kill rares &c. for them, because they can be purchased from the renown envoys e.g. [npc=200566] for 50 [currency=2003].
