TL:DR - It seems like you have to complete the War Within max level campaign, A Light in the Dark, as well as the 11.0.7 Story Campaign, Lingering Shadows, before you can speak to Khadgar and start this questchain. It seems that you need to have completed these storylines ON THE SAME CHARACTER.
Long story - I was confused like many others when I went to start this quest and couldn't see Khadgar. All that I could see was Alleria's quest, Lingering Shadows. So, I got on my hunter, who I used to complete Lingering Shadows, and while that allowed me to see his quest on the world map, I still could not see Khadgar. So, I got on my Death Knight, who I used to complete the max level storyline, months ago. I could now see Khadgar, but the only quest available was Alleria's Lingering Shadows. After running through that storyline for the second time, now using my Death Knight, I was able to see Khadgar AND pick up his quest, Survivor's Guilt. So, at least for now, it seems that the credit for the completion of these quests is character specific, not warbound.

This is super annoying. I have completed the max level campaign on one character, and Lingering Shadows on another, so now I need to complete either of them on the opposite character to see these quests? Very strange given the 'Warband' theme of this expansion. This kind of progress ought to be account-wide.
I see quest icon, but Khadgar is not in Foundation Hall to pick up quest. He must be cruising around the city in his fancy wheelchair.

Description
We have all suffered from Xal'atath's deeds, some more than others. I am glad to be alive, even if I now feel as old as I look. <Khadgar tries to laugh but coughs instead. He waves off your worry.> Don't worry about me. But Aethas... I've heard he went back to Dalaran's crash site. And that he hasn't slept or eaten since we arrived. He wants my help, but I haven't been able to bring myself to visit the crash site. Will you go in my stead?
